Listed below are types of forms commonly used by Athletic Commissions and other athletic organizations. These specific versions were developed by the members of the ARP to provide Athletic Commissions and Amateur Organizations templates with which to upgrade or improve specific forms.
These forms are listed in a Word document format so that Physicians and Commissions may “cut and paste” or modify any sections they deem appropriate, and also in a PDF format so they can be easily saved and printed as is.
These forms are not intended to imply or establish any medical standard of care or other guidelines.
Please note that the Medical History form is intended to be printed on the reverse side of the Licensing Medical Exam, and on the reverse side of the Pre/Post-Bout Physical Exam.
